This study compares estimating blood pressure directly from PPG with pipelines that first reconstruct ECG from PPG. In MIMIC-III, PPG was more strongly linked to arterial pressure (|r| = 0.247) than ECG was (r = 0.018). Across 1.74M segments from 3,127 patients, direct PPG-to-BP prediction met British Hypertension Society grade A, with MAE of 4.82 mmHg systolic and 4.31 mmHg diastolic, beating every ECG-mediated pipeline.
