CardioNets translates 12-lead ECG into cardiac MRI-level measurements and synthetic MRI images. It combines cross-modal contrastive learning with a masked autoregressive image generator and was trained on 159,819 samples from five cohorts, including UK Biobank, with external validation on 3,767 cases. In UK Biobank it improved cardiac phenotype R² by 24.8% and cardiomyopathy AUC by up to 39.3% over baselines, and in a reader study it was 13.9% more accurate than physicians using ECG plus real MRI.
